Lower eyelid "bags" or puffiness is due to bulging of fat from the eye socket as well as atrophy of some of the facial fat along the cheek and rim of the eye socket.  Here a hidden incision was made through the inside of the lid and her fat was redisrtibuted along the rim of the orbit (eye socket). This flattened the visible bulges and filled the under eye hollows
